FOR FIRST-TIME PATIENTS
All new patients are required to complete Service Level 1 - Initial Assessment. For this service, please do the following:
- Fill out and submit four forms:
Please be advised that if you leave the site prior to completion of any form, all information entered will not be saved for that form. It is also important that you remember to hit the submit button once you have finished filling out a form.
2. Please send us a copy of the front and back of your medical insurance card. Select any one of 3 ways to do this:
- Fax us a copy to 1-888-204-5975
- Send us an e-mail with an attached copy to insurance@geneticcounselingservices.com
- Take a picture with your phone and then e-mail it to us.at insurance@geneticcounselingservices.com
Once we have received your submitted forms a copy of your insurance card, we will contact you to schedule you for a telephone consultation appointment with our genetic counselor. If you have any questions or need assistance with anything, please do not hesitate to contact us.
